Changes to National Health Insurance Eligibility Certificates from August
〈 8月から国民健康保険資格確認書などが変わります 〉

National Health Insurance Eligibility Certificates

People who have registered their My Number Card as their health insurance certificate(※):
Notification of eligibility information; A4 size plain paper
People who have not registered their My Number Card as their health insurance certificate:
Eligibility certificate light purple → [New] light yellow-green card
To be Sent:
Mailed to head of household by July 31
With the discontinuation of issuance of conventional insurance certificates, we will issue an Eligibility Certificate to those who have not registered their My Number Card as their health insurance certificate, and a Notification of Eligibility Information to those who have registered their My Number Card as their health insurance certificate.
However, for the Notification of Eligibility Information, as a general rule, there will be no expiration date or information on the percentage of costs to be borne for people under the age of 70, and if there are no changes or updates to the information already provided, no new notification will be issued.

National Health Insurance Ceiling-Amount Application Certificate

Those aged 69 or younger:
Those who wish to continue using their certificate must complete the renewal process.
Please contact the national health insurance team of your respective ward.
※Certificates applied for by the end of August are valid from August 1.
Those aged 70 to 74:
  1. Households with individuals aged 70~74 enrolled in the National Health Insurance system and have a taxable income between ¥1.45 million and ¥6.9 million.
  2. Members of households where all members are exempt from municipal tax.
    A notice will be sent to individuals who meet either of the above conditions by late July. Those who believe they are eligible but have not received a notice by the end of July should contact the national health insurance team of their respective ward.

We will send an eligibility certificate (which will replace your existing health insurance card) to everyone who is insured, regardless of whether or not they have registered their My Number card as their health insurance card, Certificate for Ceiling-Amount Application and Reduction of the Standard Amount of Patient Liability for Advanced-Age Senior Citizens
The issuance of certificates was discontinued on December 2, 2024. Currently, those who hold certificates will also have their qualifications listed on their eligibility certificate.

Confirmation Letters in Late July for Those Eligible for Insufficient Tax Rebate Supplementary Payments (supplementary fixed tax reductions)
〈 不足額給付（定額減税を補足する給付）7月下旬に、対象者へ支給確認書を送付します 〉

Overview:
The supplementary tax rebate is a payment made to individuals who are eligible for the fixed-amount tax reduction(※), meaning people whose fixed-amount tax reduction amount exceeded the 2024 fiscal year income tax amount or the 2024 fiscal year individual resident tax income tax amount prior to the fixed-amount tax reduction, and who could not fully receive the tax reduction, resulting in an insufficient amount in the initial adjustment payment made in the 2024 fiscal year.
※Only tax-paying individuals with a total income of 18,050,000 yen or less are eligible.
※If your income tax and the income portion of your individual resident tax for the 2024 fiscal year are both 0 yen before the flat-rate tax reduction, you are not eligible for the flat-rate tax reduction.

Who is eligible and the application method

You have received the initial rebate for fiscal year 2024, and you are eligible for the supplementary tax rebate.
No application required
The supplementary tax rebate will be transferred to the same bank account as the initial rebate in mid-September. Please check the rebate confirmation letter for the payment amount and details for the account it will be transferred to.
If you want to change the account for the transfer, please contact the Special Benefits for Residence Tax-Exempt Households Call Center, below, by Friday, August 29, 2025.
You have not yet received the initial rebate for fiscal year 2024, and you are eligible for the rebate.
Application required
Please confirm the amount of the rebate on the rebate confirmation letter, fill in your bank account details, and submit the letter. Please be sure to do this by the deadline.
