【National Health Insurance Ceiling-Amount Application Certificate】
〈People under 70 years old〉
People who wish to continue using their certificate must complete the renewal process.
Please contact the Senior Citizens Support and Health Insurance Division of your local ward office (Certificates
applied for by the end of August will have a validity starting August 1.)
〈People aged 70 to 74〉
Conditions:
[1] Members of households with people (aged 70~74) insured by National Health Insurance and with a taxable income of between ¥1.45 million and ¥6.9 million.
[2] Members of households where all members are exempt from municipal tax.
If one of the above conditions applies to the applicant, then new certificates will be sent by late July. If you do not receive your certificate by July 31, please contact the Senior Citizens Support and Health Insurance Division of your local ward office.
※ The name of some wards will change from January 1, 2024 due to ward reorganization. Insurance cards and certificates mailed at this time can still be used after January 1, 2024.

Eligibility:
Costs incurred between April 1, 2023 and January, 31, 2024 to acquire housing, renovate, rent, or move home.
Conditions:
(1) Couples whose marriage registration was accepted between March 1, 2023 and January 31, 2024
(2) Couples where both people were 39 years of age or younger on the date of marriage.
(3) Their total household income for 2022 was under 5 million yen, and other conditions.

(2) Welfare Allowance for Children with Disabilities (Shōgaiji fukushi teate)
People, aged 20 and younger, with severe disabilities who require constant care.
(3) Special Child-Rearing Allowance (Tokubetsu jidō fuyō teate)
Parents who are raising children (aged 20 and younger) with disabilities which require long-term care.
※Applicants currently receiving allowances are required to submit an income status report in the August of every year. Further information will be sent by post to eligible households.
